More details have emerged on Dylan O’Brien’s injury on the set of his new movie, Maze Runner: The Death Cure. The actor was injured while filming a scene in Vancouver this past March.

There was a lot of confusion surrounding what exactly had happened to Dylan on set, but now it seems we may finally have an answer. The Globe and Mail has obtained records from WorkSafe B.C. describing what exactly caused his injuries.

Apparently the report states, “The incident occurred when the rigger let out rope to try to prevent the actor from being pulled off the leading vehicle, but ran out of rope length. This resulted in the worker being pulled off the leading vehicle, striking the ground due to the length of rope and then being partially suspended in front of the trailing vehicle. This resulted in a serious injury to the worker.”

The record goes on to say, “changes were made to a ‘planned and rehearsed’ action scene on March 17 in an effort to shoot the scene in one sequence instead of two. ‘When the change was made to the sequence, there was no meeting held to review the changes with all involved. The worker performing the action sequence received minimal instruction on this new action sequence.’”

How scary! As of right now Dylan is recovering from his injuries and production on the film will resume later this year.
